New Patient Registration
Welcome to Champion Wood Animal Hospital!
Before you come into our clinic, we ask that you complete our New Client Form.
We ask for basic information like the age and breed of your pet, as well as more detailed information like their medical conditions and vaccination history. Please complete this form with your and your pet's basic information so that we may effectively and efficiently provide the service you deserve!
It’s very helpful if you have medical records from your previous veterinarian so we can get a fuller picture of your pet’s medical history.
We are looking forward to developing a long-lasting relationship with you and your family.

Physical Exams
New patients will have a comprehensive physical exam during their first visit.
We’ll look at everything from head to tail, with no paw left unturned. This exam may also include bloodwork.
We’ll go over your pet’s vaccinations and medical history, as well as ask questions about any changes you’ve noticed in your pet’s mood, behavior, or eating habits.
We pride ourselves in taking our time to get to know both you and your pet. We’ll answer any questions or concerns you may have and work with you to plan ongoing care for your pet to ensure a happy, healthy life.
